With free trade under the African Continental Free Trade Area Agreement underway, role players anticipate that South African pharmaceutical manufacturers will perform an increasingly important role in the provision of vaccines and other pharmaceutical products to the rest of Africa.\r\n\r\nIn the retail space, analysts foresee continued expansion of ecommerce and other dispensing channels, such as pharmaceutical vending machines. The domestic pharmaceutical market, which generated total revenue of approximately R56.4bn in 2019, is a supplier of essential products that improve health and save lives and has been identified by government as a strategic sector. South Africa’s antiretroviral (ARV) programme is the largest of its kind globally and the country is one of the world’s major producers of radiopharmaceuticals0F . The importance of the pharmaceutical sector has been amplified by the coronavirus pandemic, which has created an unprecedented global health crisis. South Africa produces the steroid dexamethasone, which has proved to be a game-changer in the treatment of severe coronavirus cases, and local researchers continue to play a key role in coronavirus vaccine trials. In the absence of a safe and effective vaccine, hand sanitisers and other hygiene products remain in high demand and the market continues to be flooded with products supplied by non-specialist companies.

Strengths

• About 70% of pharmaceutical products consumed in South Africa are locally produced.

Pandemic-related supply chain disruptions have highlighted the risks associated with South Africa’s overreliance on imported pharmaceutical inputs and certain finished medicinal products, amplifying the need for greater self-sufficiency. Role players have welcomed government’s commitment to support locally manufactured products and are optimistic that local manufacturers will be able to ramp up production to meet the needs of the country and the continent. South African-based pharmaceutical manufacturers are set to benefit from the creation of the African Continental Free Trade Area and, with free trade between African states scheduled to commence in 2021, role players are hoping to expand pharmaceutical exports into the rest of Africa. \r\n\r\nIn the wholesale and retail markets, stakeholders predict that players will continue to explore non-traditional distribution, delivery and dispensing models. Ecommerce is expected to remain on a strong growth trajectory. With private retail pharmacies already partnering with the Department of Health to dispense antiretroviral (ARV) treatment and chronic medications to state-patients, role players anticipate a higher level of collaboration between the public and private sectors going forward. In the research and development space, stakeholders are hopeful of attracting investment for the development of new treatments and hygiene products. In the interim, the quest for safe and effective coronavirus vaccines and other treatments continues. The growth in the global pharmaceutical industry is expected to be between 3% and 6% CAGR, reaching a value of approximately $1.2 trillion in 2016. For the local industry, growth is expected because of increasing government support for the use and manufacture of pharmaceuticals. The future is “very positive from a generic point of view” according to Vivian Frittelli, CEO of the National Association of Pharmaceutical Manufacturers and the generic manufacturing industry is expected to grow by 10% in 2014 and 10% to 12% annually up to 2019. This is because of government’s increasing support for the use of generics for the treatment of HIV/AIDS and TB and the expiry of the patents for a number of originator drugs. \r\n \r\nIn the short term, however, the deteriorating macroeconomic outlook is likely to slow the consumption of pharmaceuticals in the private healthcare and retail pharmaceutical sector and impact on the costs of imported ingredients and products. Manufacturers are therefore likely to look for opportunities outside South Africa to drive volumes and profits.
