Report Coverage

This report on the pharmaceutical industry in South Africa provides comprehensive information on the manufacturing, wholesale and retail subsectors, price regulation, medical cannabis, market shares, notable multinational and local players and corporate actions. There are profiles of 128 companies including local dominant players Aspen Pharmacare and Adcock Ingram, multinationals such as Bayer, GlaxoSmithKline and Pfizer and retailers Clicks and Dis-Chem and other players such as Nativa.

Introduction

• More than 80% of South African pharmaceutical manufacturing and distribution revenue is from the private sector. \r\n
• State purchases of pharmaceuticals are declining, and manufacturers’ margins are under pressure due to low regulated price increases for scheduled pharmaceuticals in a high inflation environment. \r\n
• In the retail sector Clicks and Dis-Chem have wholesale volume advantages and an ever-increasing footprint. \r\n
• Consumer spending patterns have forced consolidation in the retail sector, the growth of private label sales, growth in convenience formats, and diversification into new services and products. \r\n
• There has been a flurry of investments in localising vaccine raw material and finished dose manufacturing, although the sustainability of these initiatives is far from secure.

Trends

• A shift from reactive medicine to preventative healthcare resulting in demand for immunity-building vitamins, tonics, minerals and supplements.
• Focus on research and development, particularly drugs and therapies targeting COVID-19, HIV/Aids, TB, malaria, cancer, and chronic diseases.
• Growing pressure on pharmaceutical companies to improve access to healthcare by making medicines more affordable.
• Increased focus on localisation, including local sourcing of inputs and the development of local manufacturing capacity.
• Recovery of demand as elective surgery and procedures and illnesses such as flu return to pre-pandemic patterns.
• Small community pharmacies are finding it difficult to compete with large retail businesses.

Opportunities

• Cannabis cultivation, processing and medical product manufacturing.
• Rapid growth in generic sales making medicine more affordable.
• Strong R&D and advanced manufacturing capabilities may attract investment-.

Challenges

• Growing concentration of wholesale and retail pharmacy supply.
• Low regulated medicine price increases in the context of high international inflation.
• Poor economic growth constraining consumer spending on healthcare.
• Reduced government budgets for medicines restricting local opportunities in the face of cheaper international competition.
• South Africa’s dependence on imported active ingredients.
• Sustaining local vaccine manufacturing in the face of declining demand.

Outlook

• The pharmaceutical industry is well developed and supported by strong R&D and advanced manufacturing capabilities, including biotechnology manufacturing facilities. \r\n
• The Department of Health’s affordable medicines programme has been instrumental in lowering scheduled pharmaceutical prices, particularly in the public sector. \r\n
• Investments in vaccine manufacture have begun to address South Africa’s exposure to vulnerabilities and dependence on active pharmaceutical ingredient and vaccine imports.\r\n
• These investments have strengthened South Africa’s position as the hub for sub-Saharan African pharmaceutical research, development, and supply. \r\n
• However, vaccine demand has dropped and doses are being donated to African countries. \r\n
• Margin pressure is expected to grow as the economy remains poor and restraints on consumer spending intensify. \r\n
• The implementation of the National Health Insurance is likely to put further pressure on manufacturers and wholesalers to reduce medicine prices.

Report Coverage

This report focuses on the pharmaceutical industry in South Africa including manufacture, retail and wholesale trade in pharmaceuticals and alternative health products, such as medicinal cannabis. The report includes information on the state and size of the industry, notable players in each subsector, developments, corporate actions, regulations and other influencing factors. There are profiles of 129 companies including international companies Cipla, Novartis, Johnson & Johnson and Pfizer, local companies including Aspen and Adcock Ingram, retailers Clicks and Dis-Chem, and cannabis companies such as Goodleaf.

Introduction

The importance of the pharmaceutical industry has been amplified by the COVID-19 pandemic and the resulting global health crisis. The sector, which generated total sales revenue of around R60.4bn in 2021, has been designated as a strategic industry as it produces and supplies essential products. South Africa has the world’s largest antiretroviral (ARV) programme and is one of the major producers of radiopharmaceuticals . With the ongoing development of local manufacturing capacity and capabilities, local pharmaceutical companies are set to play a pivotal role in the production and supply of COVID-19 vaccines and other medicines for the domestic market and the rest of Africa. \r\n\r\nThis report also examines the wholesale and retail trade in pharmaceuticals and alternative health products, such as medicinal cannabis. In 2021, government unveiled a cannabis master plan that aims to expand the country’s nascent cannabis industry, attract foreign investment and create up to 25,000 jobs. While cannabis is widely touted as a panacea, medical scientists caution that there is still a lack of reliable evidence to support claims about its medical efficacy.

Strengths

• South Africa has the largest antiretroviral (ARV) treatment programme in the world and almost half of the people infected with HIV/Aids in South Africa are on ARVs.
• South Africa is one of the world’s largest producers of radiopharmaceuticals.
• South Africa is the only Southern African Development Community member country that complies with the World Health Organisation’s Good Manufacturing Practice standards.
• The industry is supported by strong research and development capabilities and advanced manufacturing capabilities, including biotechnology manufacturing facilities.
• The presence of several of the world’s largest pharmaceutical companies supports research and product development.

Weaknesses

• Although pharmaceuticals are strictly regulated, over-the-counter medications and prescription drugs are widely misused and abused.
• Inadequate quality control of CBD products, hand sanitisers and other hygiene products.
• The cost of medicinal products, particularly newly developed treatments such as cell and gene therapies, is high.
• There are around 30 pharmacists per 100,000 people, well below the global benchmark of 50 per 100,000.
• There is an overreliance on imported active pharmaceutical ingredients and certain finished medicinal products.

Opportunities

• Advances in 3D printing have made the production of small batches of tablets viable.
• As the Department of Health expands its chronic medication delivery and collection services programme, greater collaboration between the public health sector and pharmacies is anticipated.
• Government has identified the pharmaceutical industry as a strategic sector and it supports the procurement of locally-manufactured products.
• National Health Insurance offers opportunities for private sector community
• Participation of retail pharmacies in the national vaccination programme.
• pharmacies to provide primary care services to public health patients.
• Strong demand for home delivery services is driving the expansion of courier pharmacy networks.
• The development and manufacture of active pharmaceutical ingredients.
• The development of traditional/indigenous medicines.
• The development, production and supply of health products containing cannabidiol, such as CBD oil.
• The supply of healthcare products via online and mobile retail channels.
• The supply of sanitising products and disinfectants locally and to the rest of Africa.
• With free trade between African counties underway, South Africa is aiming to expand its pharmaceutical exports into the rest of Africa.

Threats

• Climate-change related shocks.
• Counterfeit medication and substandard products pose health risks.
• Further reputational damage resulting from the misuse/abuse of prescription drugs.
• Global supply chain disruptions.
• Misinformation spread via social media and other channels about vaccinations and unproven remedies.
• New COVID-19 variants and the outbreak of other pandemics.
• Social unrest.
• Sustained economic pressures could result in funding constraints.
• The emergence of drug-resistant super bugs.

Outlook

The pandemic has brought into sharp focus the critical need for access to vaccines and affordable medicines. The gridlocked global supply chain has highlighted the risks associated with South Africa’s over-reliance on imports for active pharmaceutical ingredients and essential drugs. While stakeholders have welcomed initiatives to boost domestic manufacturing capacity and capabilities, they say that South Africa needs to reduce its dependency on imported inputs. With free trade under the African Continental Free Trade Area Agreement underway, role players anticipate that South African pharmaceutical manufacturers will perform an increasingly important role in the provision of vaccines and other pharmaceutical products to the rest of Africa.\r\n\r\nIn the retail space, analysts foresee continued expansion of ecommerce and other dispensing channels, such as pharmaceutical vending machines. Stakeholders say that the collaboration of private retail pharmacies with the Department of Health on the COVID-19 vaccination programme has proved highly successful and paves the way for future partnerships between the public and private sectors. While the pandemic presents opportunities for companies in the pharmaceutical value chain, analysts expect the operating environment to remain challenging over the medium-term. \r\n\r\nAnalysts predict that the local medical cannabis industry will expand rapidly over the next five years. While some smaller cannabis businesses have reservations about large pharmaceutical companies dominating the market, others say that the industry has been given a “shot in the arm” by government’s cannabis master plan.

Report Coverage

This report focuses on the manufacture of pharmaceuticals in South Africa and includes the wholesale and retail trade in pharmaceutical products. It includes information on the state and size of the sector and influencing factors including the response to coronavirus, regulation, research and development and competition. There are profiles of 122 companies including international companies operating in South Africa such as Cipla, Sanofi and Novartis, major local manufacturers such as Aspen and Adcock Ingram and retailers including Clicks and Dis-Chem. Introduction

This report focuses on the manufacture of pharmaceuticals in South Africa and examines the wholesale and retail trade in pharmaceutical products. The domestic pharmaceutical market, which generated total revenue of approximately R56.4bn in 2019, is a supplier of essential products that improve health and save lives and has been identified by government as a strategic sector. South Africa’s antiretroviral (ARV) programme is the largest of its kind globally and the country is one of the world’s major producers of radiopharmaceuticals0F . The importance of the pharmaceutical sector has been amplified by the coronavirus pandemic, which has created an unprecedented global health crisis. South Africa produces the steroid dexamethasone, which has proved to be a game-changer in the treatment of severe coronavirus cases, and local researchers continue to play a key role in coronavirus vaccine trials. In the absence of a safe and effective vaccine, hand sanitisers and other hygiene products remain in high demand and the market continues to be flooded with products supplied by non-specialist companies.

Strengths

• About 70% of pharmaceutical products consumed in South Africa are locally produced.
• South Africa is one of the world’s largest producers of radiopharmaceuticals.
• South Africa’s HIV/Aids antiretroviral (ARV) programme is the largest of its kind in the world.
• The presence of several of the world’s largest pharmaceutical companies facilitates knowledge sharing and supports research and product development.
• The South African pharmaceutical industry is well developed and complies with World Health Organisation production standards.
• There is a high standard of research and development.

Weaknesses

• Although pharmaceuticals are strictly regulated, over-the-counter medications and prescription drugs are widely misused and abused.
• Quality control relating to hand sanitisers and other hygiene products.
• The cost of medicinal products, particularly newly developed treatments such as cell and gene therapies, is high.
• There are around 26 pharmacists per 100,000 people, well below the global benchmark of 50 per 100,000.
• There is an overreliance on imported active pharmaceutical ingredients.

Opportunities

• Advances in 3D printing have made the production of small batches of tablets viable.
• As the Department of Health expands its chronic medication delivery and collection services programme, greater collaboration between it and pharmacies is anticipated.
• Government has identified the pharmaceutical industry as a strategic sector and it supports the procurement of locally manufactured products.
• Strong demand for home delivery services is driving the expansion of courier pharmacy networks.
• The development of bio-based medicinal products using locally sourced plant-based ingredients.
• The development, production and supply of health products containing cannabidiol.
• The National Health Insurance offers opportunities for private sector community pharmacies to provide primary care services to public health patients.
• The supply of complementary (alternative) healthcare products via online and mobile retail channels.
• The supply of sanitising products and disinfectants to the domestic market and to the rest of Africa.
• With free trade between African counties set to commence in 2021, South Africa aims to expand its pharmaceutical exports into the rest of Africa.

Threats

• Climate change-related shocks.
• Counterfeit medication and substandard sanitising products pose a threat to public health and safety.
• Economic pressures could result in funding constraints.
• Further reputational damage resulting from the misuse/abuse of prescription drugs.
• The emergence of drug-resistant “super bugs”.
• The pandemic could cause further supply chain disruption.
• The resurgence of coronavirus and the emergence of new infectious diseases.

Outlook

Pandemic-related supply chain disruptions have highlighted the risks associated with South Africa’s overreliance on imported pharmaceutical inputs and certain finished medicinal products, amplifying the need for greater self-sufficiency. Role players have welcomed government’s commitment to support locally manufactured products and are optimistic that local manufacturers will be able to ramp up production to meet the needs of the country and the continent. South African-based pharmaceutical manufacturers are set to benefit from the creation of the African Continental Free Trade Area and, with free trade between African states scheduled to commence in 2021, role players are hoping to expand pharmaceutical exports into the rest of Africa. \r\n\r\nIn the wholesale and retail markets, stakeholders predict that players will continue to explore non-traditional distribution, delivery and dispensing models. Ecommerce is expected to remain on a strong growth trajectory. With private retail pharmacies already partnering with the Department of Health to dispense antiretroviral (ARV) treatment and chronic medications to state-patients, role players anticipate a higher level of collaboration between the public and private sectors going forward. In the research and development space, stakeholders are hopeful of attracting investment for the development of new treatments and hygiene products. In the interim, the quest for safe and effective coronavirus vaccines and other treatments continues. Researchers say that until such time as a reliable vaccine has been administered to the population, hand sanitisers will remain a vital aspect of the pandemic response.

Introduction

This report focuses on the pharmaceutical sector, which researches, develops, markets and distributes drugs, as part of the wider healthcare system. The industry is valued at R50bn at manufacturers’ exit price and R68bn using retail sales. The challenge facing the industry is expanding access to and affordability of drugs, on the one hand and growing South Africa’s drug manufacturing capacity on the other. The sector seeks to meet this challenge in the context of implementation of the National Health Insurance (NHI) and rapid technological developments.

Strengths

• Investments in capabilities to conduct clinical trials, secondary and tertiary manufacturing.
• Many different pharmaceutical companies operating in specialised markets.
• South Africa has a high demand for pharmaceutical products.

Weaknesses

• Dependence on imported active pharmaceutical ingredients and pharmaceuticals.
• Skills shortages remain high, with 12,000 pharmacists needed to meet the international benchmarks.
• Uncertainty over the implementation of National Health Insurance.

Opportunities

• Export into growing markets in Africa.
• Improvements in regulatory environment with the South African Health Products Regulatory Authority offering an improved service.
• Intellectual property landscape supportive of more affordable drugs and local manufacturing.
• Local manufacturing capabilities can be leveraged to serve growing market demand in the rest of Africa and globally.

Threats

• Continued closures of independent pharmacies.
• Debt in larger companies may limit expansion and may result in the closure of units or even entire businesses.
• Increasing regulatory costs.
• Low economic growth and currency volatility.

Outlook

The outlook for the pharmaceutical industry in South Africa is challenging. Since 2017, high debt in major companies has necessitated restructuring and the sale of businesses, and this process is likely to continue. Government support for pharmaceutical manufacturers, price setting for medications and investments in special economic zones represents an opportunity for the growth of local manufacturing capacity, although this is contingent on government and the private sector meeting their commitments. In providing support, government is hoping to grow local manufacture of drugs and bring down the cost of medicines. The changes to the intellectual property regime, vigilance of the Competition Commission and activism in civil society continue to point to a framework that potentially supports a reduction in drug prices and builds local manufacturing capacity.

Report Coverage

The detailed report on the Pharmaceutical industry focuses on developments in the South African manufacturing and retail sectors during 2016 and 2017. The report profiles 88 companies including newcomer Inicio, which recently secured a licensing agreement with German firm Fluxpharm. The agreement is a key step towards possible domestic production of active pharmaceutical ingredients (APIs) for HIV/AIDS drugs, which locally-based pharmaceutical manufacturers currently import from China and India. If this is successful, South Africa will have its first black-owned pharmaceutical company. State-owned pharmaceutical manufacturer Ketlaphela has yet to begin production of APIs. Also profiled are three companies that have been investigated by the Competition Commission in relation to the supply of various cancer medicines, Pfizer, Aspen and Roche and its US-based biotechnology company, Genentech.

Introduction

This report focuses on developments in the South African pharmaceutical industry during 2016 and 2017. The pharmaceutical industry provides the vital service of providing medication and was valued at R42.6bn in 2016. The most important challenges facing the industry are tensions between improving access to affordable medicines on the one hand, and growing the manufacturing of pharmaceuticals on the other. Other factors influencing the sector include the introduction of the National Health Insurance (NHI), significant changes in technologies to manufacture drugs and the emergence of the use of cannabis and other alternative healing methods.

Strengths

• Improving levels of local manufacturing capacity.
• Investments in capabilities to conduct clinical trials, secondary and tertiary manufacturing.
• Many different pharmaceutical companies operating in specialised markets.
• South Africa has a high demand for pharmaceutical products.

Weaknesses

• Dependence on imported APIs and pharmaceuticals.
• Single Exit Price (SEP) mechanism means manufacturers are pressurised as far as pricing is concerned.
• Skills shortages remain high, with 12,000 pharmacists needed to meet international benchmarks.

Opportunities

• Export into growing markets in Africa.
• Improvements in regulatory environment with the launch of the South African Health Products Regulatory Authority (SAHPRA).
• Improvements in the Intellectual Property (IP) landscape.
• Local manufacturing capabilities can be leveraged to serve growing market demand in the rest of Africa and globally.
• The establishment of API manufacturing sites.
• The state pharmaceutical company, Ketlaphela, could reduce prices and support research once it is operational.

Threats

• Continued closures of independent pharmacies.
• Increasing regulatory costs.
• Low economic growth and currency volatility.
• The legalisation of cannabis as a natural medication.

Outlook

Growth in the pharmaceutical industry is likely to continue, but at a slower pace than experienced over the last five years. Role players believe that the South African pharmaceutical industry will continue to experience major changes mainly in meeting the objectives of providing affordable drugs and at the same time growing manufacturing capacity. Reaching these objectives will prove difficult because of the low levels of economic growth, with stakeholders expressing concern regarding the impact this will have on government resources in general, and specifically on the implementation of the NHI. The interaction between an organised civil society lobbying for affordable drugs, and an organised established pharmaceutical sector seeking to protect its patent laws, is expected to play a significant role in determining the future of the pharmaceutical industry in South Africa.

Report Coverage

The report on the Pharmaceutical industry describes merger and acquisition activity, the latest regulatory developments and discusses factors influencing the success of the sector. The report profiles 90 companies ranging from South Africa’s two largest pharmaceutical manufacturers, Aspen Pharmacare and Adcock Ingram to newcomers to the Top 20, Mylan and Valeant Pharmaceuticals which both have less than 2% share of the market. Also profiled are African Medicines (Pty) Ltd t/a Pakmed Self Medication and GNLD International (Pty) Ltd, companies that use the services of contract manufacturers such as Wrapsa. Large national retailers with in-house pharmacies like Clicks and Pick ‘n Pay, which have proved to be fierce competition to traditional independent and community pharmacies, are also profiled in this report.

Introduction

This report focuses on developments in the South African pharmaceutical industry during 2015 and 2016. This vital industry, which incorporates research and development, manufacture, import, distribution, wholesale and retail of pharmaceutical products, was valued at R39.3bn in 2015, with total counting units reaching 51.9 billion. Although pharmaceutical companies acknowledge that their primary responsibility is to ensure access to affordable healthcare for consumers, some stakeholders, especially those campaigning for a change in the patent laws and in the private healthcare sector, believe companies continue to place large profits before improved health.

Strengths

• Active government support for industrialisation efforts.
• Strong and active civil society organisations focused on improving access to and affordability of drugs.
• There is a strong pharmaceutical R&D and clinical trial sector.
• Well established local industry, with strong international presence.

Weaknesses

• Currently, dependence on MCC and a long registration period for pharmaceutical products.
• Dependence on imported APIs and pharmaceuticals.
• Patent system for drugs consistently challenged due to high cost of pharmaceuticals.
• SEP regulation which does not allow manufacturers to increase prices to adjust for higher input costs.
• Shortage of pharmacists and pharmacist assistants.

Opportunities

• Export into growing markets in Africa.
• Growth of generic sector.
• Improvements in procurement systems for public sector drug provision.
• Improvements in regulatory environment with the imminent launch of the South African Health Products Regulatory Authority.
• R&D, particularly the “Farmer to Pharma grand challenge.”
• Well-functioning state-run pharmaceutical company could reduce prices and potentially support research.

Threats

• Continued closures of independent pharmacies.
• Low economic growth and currency volatility.
• Regulatory costs are high for alternative and complementary medicines.

Outlook

The South African pharmaceutical industry is expected to experience significant changes over the next few years. These changes include the introduction of a new regulator, the South African Health Products Regulatory Authority (SAHPRA), the implementation of the National Health Insurance and continued focus on industrialisation efforts in the pharmaceutical industry. These developments coupled with a strong civil society advocacy for changing the patent laws, are likely to create an environment that is ripe for change. These changes potentially provide South Africa with a basis to grow the pharmaceutical industry and at the same time reduce the costs of drugs. The growth of the generics segment of the market in particular is likely to provide a basis for growing manufacturing capacity, and at the same time increasing access to life-savings drugs. Stakeholders assert, however, that industry and government will need to cooperate on these matters, as the reforms being undertaken are large and carry with them significant challenges.

Introduction

This report focuses on the South African pharmaceutical industry. The manufacturing industry generated R32.4bn while the retail industry in pharmaceutical and medical goods, cosmetics and toiletries reached R49.96bn in 2013. Generic drug use is increasing rapidly, driven by government’s goal for more affordable pharmaceuticals in South Africa. The domestic manufacturing industry is relatively small and 65% of pharmaceuticals are imported. Medical aids have increasing power, shaping consumer use of pharmaceuticals to the advantage of corporate pharmacies, which have been able to grow their share of the market to 35.5% since 2004.

Strengths

• Pharmaceuticals are a vital part of the healthcare system.
• The two leading pharmaceutical companies are locallybased,\ngeneric manufacturers.
• There are a variety of different types of pharmaceutical\ncompanies present in the country.
• There is a strong pharmaceutical R&D and clinical trial sector.

Weaknesses

• Critical skill shortage of pharmacists and pharmacist\nassistants.
• Dependence on imported APIs and pharmaceuticals.
• Dependence on MCC and a long registration period for\npharmaceutical products.
• SEP regulation which does not allow manufacturers to increase prices to adjust for higher input costs

Opportunities

• Establishment of SAHPRA which is to replace the MCC.
• Increasing roll-out of government tenders for generic drugs\nto treat HIV/AIDS and TB.
• NHI through which a large number of South Africans will be\ngiven access to pharmaceuticals.
• Opportunities for API and pharmaceutical manufacturing\nexist.
• Patent expiry opens the market for the development of\ngenerics.
• Potential for generic manufacturers to utilise their spare\ncapacity to produce products to export globally.
• Potential for increased exports to African countries.

Threats

• Continuing delays in the registration of pharmaceutical product at the MCC.
• Deteriorating economic conditions.
• For community pharmacists, the practice of DSP.
• Increasing number of cheap imports from India and China.
• New regulations for complementary and alternative medicines will mean high costs which may be prohibitive for small companies.
• Rising input costs, especially rising cost of imported APIs.

Outlook

The growth in the global pharmaceutical industry is expected to be between 3% and 6% CAGR, reaching a value of approximately $1.2 trillion in 2016. For the local industry, growth is expected because of increasing government support for the use and manufacture of pharmaceuticals. The future is “very positive from a generic point of view” according to Vivian Frittelli, CEO of the National Association of Pharmaceutical Manufacturers and the generic manufacturing industry is expected to grow by 10% in 2014 and 10% to 12% annually up to 2019. This is because of government’s increasing support for the use of generics for the treatment of HIV/AIDS and TB and the expiry of the patents for a number of originator drugs. \r\n \r\nIn the short term, however, the deteriorating macroeconomic outlook is likely to slow the consumption of pharmaceuticals in the private healthcare and retail pharmaceutical sector and impact on the costs of imported ingredients and products. Manufacturers are therefore likely to look for opportunities outside South Africa to drive volumes and profits.
